    These Terms of Service ("Terms") govern your use of Aequis telehealth services. By using our services, you agree to be bound by these Terms.

    Telehealth Services

    Aequis provides telehealth-based hormone optimization and related medical services. Our services include virtual consultations with licensed physicians, personalized treatment plans, prescription medications, and ongoing care management.

    Eligibility

    To use our services, you must:

    • Be at least 18 years of age
    • Be a resident of a state where we are licensed to practice
    • Provide accurate and complete health information
    • Have a valid form of identification

    Medical Disclaimer

    Our services are not intended to replace emergency medical care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. Our physicians will determine if telehealth services are appropriate for your condition.

    Patient Responsibilities

    As a patient, you agree to:

    • Provide accurate and complete health information
    • Follow your treatment plan as prescribed
    • Attend scheduled appointments and lab work
    • Report any adverse reactions or changes in your condition
    • Keep your account information secure and confidential
    • Pay for services in a timely manner

    Prescription Medications

    Prescriptions are issued at the sole discretion of our licensed physicians based on their clinical judgment. We cannot guarantee that any specific medication will be prescribed. You must use prescribed medications only as directed.

    Payment Terms

    Payment is required at the time of service. We accept major credit cards and HSA/FSA payments. Refund policies vary by service type and are detailed at the time of purchase.

    Limitation of Liability

    To the maximum extent permitted by law, Aequis shall not be liable for any indirect, incidental, consequential, or punitive damages arising from your use of our services. Our total liability is limited to the fees paid for services.

    Termination

    Either party may terminate services with 30 days written notice. We may terminate services immediately if you violate these Terms or if continuing care would not be in your best interest.

    Governing Law

    These Terms are governed by the laws of the State of Delaware. Any disputes will be resolved through binding arbitration in accordance with the American Arbitration Association rules.
